If your dentist recommends in-office sedation and you choose to move forward, they’ll send your contact information directly to our team.

Complete your forms & get informed.

You’ll receive an email from us with a link to your medical forms. Once submitted, visit our Patient FAQ to learn more.

Set your sedation date.

After we receive your forms, we’ll notify your dental office so they can schedule your appointment. One of our physicians may follow up with questions after reviewing your medical history.
